Résumé

Les critères actuellement admis pour le diagnostic de la maladie cœliaque sont histologiques et comportent la constatation d’une atrophie ou une subatrophie villositaire sous régime contenant du gluten. Le diagnostic de certitude de cette affection n’est cependant obtenu qu’après régression des lésions histologiques sous un régime sans gluten strict. Ainsi, le but de ce travail est de vérifier sur des coupes histologiques le degré d’atteinte de la structure épithéliale de sujets cœliaques en phase active de la maladie et la capacité de recouvrement de l’integrité épithéliale après restriction au gluten. Des biopsies jéjunales ont été prélevées dans un but diagnostic chez 16 sujets cœliaques. Elles ont été effectuées à différents stades. Les fragments intestinaux étaient fixés au Bouin aqueux puis inclus dans la paraffine, coupés, déparaffinés et colorés à l’hémalunéosine. Selon leur aspect, ces coupes étaient classées en cinq grades. Également biopsisés 9 sujets formant le groupe témoin dans les mêmes conditions mais pour des raisons autres que la maladie cœliaque. Tous nos sujets témoins présentaient une histologie de type grade I. En phase active de la maladie ; 37,5 % des sujets malades étaient de grade V 50% étaient de grade IV et 12,5 % , étaient de grade III. En phase d’exclusion du gluten ; 6,25 % des sujets malades restaient en grade V 37,5 % en grade IV 37,5 % présentaient un grade III et 18,75 % un grade II. En , , phase de guérison ; le grade IV persistait chez 12,5 % des sujets malades, le grade III chez 25 %, en revanche, 56,25 % des sujets malades présentaient un grade II et 6,25 % un grade I. À l’issue de cette étude, il apparaît que le degré de récupération de l’épithélium intestinal diffère en fonction de chaque individu, probablement en rapport avec sa constitution, sa génétique et d’autres facteurs ; de même, il n’existe pas de durée standard quant à la période de restriction au gluten.

Abstract

The currently allowed criteria for the diagnosis of the cœliac disease are histological and comprise the observation of an villous atrophy or a subatrophy under a diet containing of the gluten. The diagnosis of certainty of this affection is however obtained only after regression of the histological lesions under a diet without strict gluten. So, the aim of this work is to check on histological cuts the degree of attack of the epithelial structure of subjects cœliac in active phase of the disease and the capacity of covering of the epithelial integrity after gluten withdrawal. Jejunal biopsies were taken aiming at a diagnostic within 16 subjects cœliac. They were carried out at different stages. The intestinal fragments were fixed at aqueous Bouin then included in paraffin, cut, dewaxed and coloured with the hémalun- éosine. According to their aspect, these cuts were classified in five ranks. Also biopsized 9 subjects forming the reference group under the same conditions but for other reasons than the cœliac disease. All our control subjects presented a histology of the type rank I. In active phase of the disease; 37,5 % of the patients subjects were of rank V 50 % were of rank , IV and 12,5 % were of rank III. In phase of exclusion of the gluten; 6,25 % of the patients subjects remained in rank V 37,5 % in rank IV 37,5 % had a rank III and 18,75% a rank , , II. In curative phase; rank IV persisted within 12,5 % of the patients subjects, rank III within 25 %, on the other hand, 56,25 % of the patients subjects had a rank II and 6,25 % a rank I. At the light of this study, it appears that the recovery degree of the intestinal epithelium is function of each individual, probably in keeping with his make-up, his genetics and other factors. Our results show that there do not exist standard duration according to the gluten withdrawal period.
